What is this survey about? Why is it important?
The aim of this survey is to understand your information needs about fertility (your ability to have children). This study is being lead by the Cancer Patient Education Network (CPEN), Canadian Chapter.
Your feedback will be used to develop patient education resources to better meet the needs of future patients and family members.
Who can take part in this study?
People who have cancer and their family members or caregivers.
What can I expect if I join the study?
If you decide to take part in this project, you will be asked to fill out one survey that will ask you questions about:
- your cancer diagnosis
- your fertility information needs
- how you would like to get information about your fertility needs
Please submit the survey even if you do not complete it.
The survey will take about 20-30 minutes to complete.
Important details about the survey:
- Participating only involves filling-out this survey.
- You do not have to take part in this project, and you can choose to stop at any time.
- You will only be asked to fill out one survey and will not be contacted after completing the survey.
- Participants will not be named or identified in any study reports, publications or presentations that may come from this study. Also, once the data is collected we will not be able to withdraw the data from the study.
- All the information you share will be kept confidential (private) and anonymous (without your name or contact details on it).
